Please download and fill out the two forms (First 3 pages of I-693), save them with your name or a unique identifier and send them back to admin@olurin.com along with ALL your available immunization records and your photo ID
It is extremely useful to include ALL your prior immunization records before you send the forms back to allow us to determine what additional tests and or vaccines you will need.
We will then reach out to you before making your in- office appointment to go over the requirements that you will need and try to start the process remotely.
